These are the requirements on how to get PWD ID/Card in the Philippines.   You will need the following

• Barangay Certificate (you can get this one from your Barangay Hall)
• Application form
• Two 1×1 ID pictures of the disabled person (this is for the medicine booklet in availing discounts)
• A duly signed clinical abstract and medical certificate signed by any licensed hospital-based physician

You can get the card for free and will expires every 3 years. This will be renewed every after 3 years, the same requirements will apply during renewal.

You will be considered a PWD (Person with Disability) if you have:

• Visual impairment
• Hearing Loss
• Orthopedic disability
• Learning disability
• Psychosocial disability
• Chronic Illness 
• Mental Disability
